Out of Network: $500 deductible 70/30 coinsurance ($2,500 out of pocket)
Prescription Plan: $15 Generic/$25 Name Brand. A 90-day supply of medication may be purchased at the drugstore for one copay.
Doctor must write the prescription for a 90 day supply of medication in order to obtain this benefit.
Employee contribution required to cover family members; see below for costs.
