  • 60-day waiting period
  • Member Services: 800 702-3862
  • In Network: $20 office visit copay;
  • Out of Network: $500 deductible 70/30 coinsurance ($2,500 out of pocket)
  • Prescription Plan: $15 Generic/$25 Name Brand. A 90-day supply of medication may be purchased at the drugstore for one copay.
  • Doctor must write the prescription for a 90 day supply of medication in order to obtain this benefit.
  • Employee contribution required to cover family members; see below for costs.
